Work Permits

Work permit applications can be printed from the Ohio Department of Commerce website (link below).

Applicants must be 14 years of age or older and have a job before starting the application process.

  1. Complete the student section of the application
  2. Have your employer complete the employer section.
  3. Have your physician complete the physician's certificate.
  4. The District will document your age from school records.
  5. Submit your forms to the Administration Offices or send via email to workpermit@wickliffecsd.org.

Only completed forms will be reviewed.  Once you have submitted your application the information will be entered into the Ohio Department of Commerce database for approval, this may take several days.

Work permits are subject to Ohio Department of Commerce regulations - ODC Minor Labor Laws
